Equitable Health Group (EHG), established in November of 2021, is a 501c3, non-profit organization that targets Georgia and Florida K-12 public and charter schools to provide economically restricted families with resources to obtain optimal health by ensuring linkage to adequate resources that fulfill a healthy and sustainable household.

At EHG, we employ a “cradle to the grave” framework that combines the needs of the various interwoven dimensions of well-being with the public health life course perspective. Resources and referrals are provided through family case management.

To achieve the vision of bridging the gap between healthy people and healthy communities, and ensuring family members are linked to appropriate care.

 

We pride ourselves on ensuring that the elected families have access to culturally and racially competent staff, as well as all referral services and resource provision in the following areas: Benefits and transportation assistance, workforce development and affordable housing.

Vision

Our vision of fulfilling a healthy and sustainable household is met by:

  • Strategic programming

  • Collaborative partnerships with local, state and government entities (i.e., Department of Motor Vehicles (DMV), Supplemental Nutrition Assistance Program (SNAP), SSI and Women, Infant, and Children (WIC)

  • Highly-skilled case managers and supporting staff will conduct needs assessments and provide specialized assistance to ensure each enrollee/client, who will be referred through K-12 school counselors and interventionists
